A cross-sectional study to ascertain what the Singapore population would regard as material risk in the anaesthesia consent-taking process and identify demographic factors that predict patient preferences in medical decision-making to tailor a more patient-centered informed consent. A survey was performed involving patients 21 years old and above who attended the pre-operative evaluation clinic over a 1-month period in Singapore General Hospital. Questionnaires were administered to assess patients’ perception of material risks, by trained interviewers. Patients’ demographics were obtained. Mann–Whitney U (...) test and Kruskal-Wallis one-way analysis of variance was used. Statistical significance was taken at p < 0.05. Four hundred fourteen patients were eligible of which 26 refused to participate and 24 were excluded due to language barrier. 364 patients were recruited. A higher level of education, being employed and younger age group are factors identified in patients who wanted greater participation in medical decisions. Gender, marital status, type of surgery, and previous surgical history did not affect their level of participation. The complications most patients knew about were Nausea, Drowsiness and Surgical Wound Pain. Patients ranked Heart Attack, Death and Stroke as the most significant risks that they wanted to be informed about in greater detail. Most patients wanted to make a joint decision with the anaesthetist, instead of letting the doctor decide or deciding for themselves. Discussion with the anaesthetist is the preferred medium of communication compared to reading a pamphlet or watching a video. Age and educational level can influence medical decision-making. Despite the digital age, most patients still prefer a clinic consult instead of audio-visual multimedia for pre-operative anaesthetic counselling. The local population appears to place greater importance on rare but serious complications compared to common complications. This illustrates the need to contextualize information provided during informed consent to strengthen the doctor-patient relationship. (shrink)

This essay is written on the following premises and argues for them. “Enlightenment” is a word or signifier, and not a single or unifiable phenomenon which it consistently signifies. There is no single or unifiable phenomenon describable as “the Enlightenment,” but it is the definite article rather than the noun which is to be avoided. In studying the intellectual history of the late seventeenth century and the eighteenth, we encounter a variety of statements made, and assumptions proposed, to which the (...) term “Enlightenment” may usefully be applied, but the meanings of the term shift as we apply it. The things are connected, but not continuous; they cannot be reduced to a single narrative; and we find ourselves using the word “Enlightenment” in a family of ways and talking about a family of phenomena, resembling and related to one another in a variety of ways that permit of various generalizations about them. We are not, however, committed to a single root meaning of the word “Enlightenment,” and we do not need to reduce the phenomena of which we treat to a single process or entity to be termed “the” Enlightenment. It is a reification that we wish to avoid, but the structure of our language is such that this is difficult, and we will find ourselves talking of “the French” or “the Scottish,” “the Newtonian” or the “the Arminian” Enlightenments, and hoping that by employing qualifying adjectives we may constantly remind ourselves that the keyword “Enlightenment” is ours to use and should not master us. Two classic learning situations are critically reviewed and interpreted from a synergetic point of view: human learning of complex skills, and animal discrimination learning. Both show typical characteristics of nonlinear phase transitions: instability, fluctuations, critical slowing down and reorganisation. Plateaus in the acquisition curves of complex skills can be viewed as phases of arrested progress in which a reorganisation of simple skills is necessary before their integration into complex units is possible. Fluctuations and critical slowing down are expressed in instances (...) of "vicarious trial-and-error," which describe the oscillating behavior of rats at a choice point that is shown only just before discrimination learning is completed. It is shown that an approach to quantum phenomena in which charged particles are treated as macroscopically extended periodic disturbances in a nonlinear c-number field, interacting with each other via massless excitations of that field, leads almost uniquely to the five basic equations of classical electrodynamics: the Lorentz force law and Maxwell's equations. ABSTRACTThis essay is speculative in character. It is the work of a historian who has completed a study, written on certain principles, of the first three volumes of Gibbon’s Decline and Fall of the Roman Empire and does not intend to advance to a similar study of the second three. He does, however, believe that such a study would differ profoundly from that he has constructed of the first trilogy and wishes to offer hypotheses as to why this should be (...) so. All hypotheses invite falsification, and he will make statements about the second trilogy and its hypothetical construction which invite research with results to which they may or may not stand up. To do this will be an exercise in the history of historiography, a sub-discipline still in progress of establishing itself. It will also give the author the opportunity of extending certain generalizations he was led to advance in writing and completing his study of Gibbon’s first trilogy, and of enquiring whether they remain valid in the light of a study of the second – given that this study is still at a hypothetical stage. AFP, whose precise function is unknown, has been classified as belonging to a protein superfamily together with (...) albumin and vitamin D‐binding (Gc) protein. AFP has been shown to bind various ligands in vitro including fatty acids, estrogens, thyroid hormones and retinoic acids. The steroid/thyroid nuclear receptor superfamily of proteins has recently become a major focus of biomedical investigation regarding regulation of gene expression. These receptors are thought to bind to DNA‐hormone response elements (HRE) that affect growth, development, differentiation, reproduction and homeostasis. The HREs are known to share DNA sequences with the various members of the nuclear receptor superfamily. In the present report, the possibility of a leucine‐zipper dimerization (heptad) motif in the carboxy‐terminal third domain of both rodent and human AFP is postulated. The presence of nine such hydrophobic repeats in the third domain of the AFP molecule mimics the heptad dimerization repeats found in the retinoic acid, thyroid, e‐erbA and other members of the nuclear receptor superfamily. Computer analysis revealed that the most conservative matching occurred between AFP and the retinoic acid class of receptors. However, other superfamily members displayed 40–60% homology with 5 of 9 AFP heptads. These findings could provide a possible mechanism for explaining the growth‐regulatory properties (both inhibition and enhancement) that have been ascribed to AFP in the last decade. (shrink)

It is recognized that Romans of the late Republic did not normally address or refer to one another by praenomen alone. Most instances in which the praenomen is used alone are easily explicable ; either the persons concerned are members of the same family, with names otherwise identical, or the praenomen itself is particularly distinctive and aristocratic.

Louis Dumont is a distinguished Indianist but his later work has undertaken to ground an allegedly general need for holism and hierarchy in comparative historical sociology. Dumont's anti‐individualist thrust, depicting as it does modern Western culture as an aberration, a kind of social disease inviting in the long run an even worse cure—the nemesis of totalitarianism— enjoyed in the 80s the status of a modern classic of sociological wisdom. Even those who, like the new humanist thinkers in France (Luc Ferry, (...) Alain Renaut, Tzvetan Todorov) fight the influential antimodern stances of Heidegger or Leo Strauss, have come to share Dumont's strictures against individualism. This paper describes the main theses of Dumont's latest book, translated into English as Essays on Individualism, while at the same time sketching a liberal critique of his anti‐individualist bias.
